Understanding cognitive ageing in healthy adults in India is essential for ensuring a good quality of life. This study aims to investigate cognitive ageing in a healthy Indian cohort, providing valuable insights into the trajectories and associations of cognitive, neurobiological, physiological, psychological, and molecular changes across the adult lifespan in India.
Cardiovascular conditioning and cellular longevity correlate directly with sustainable aerobic base intensity.** Applying physiological guidelines to Understanding Cognitive Ageing in Healthy Adults in India, sustaining training within 60% to 70% of maximum heart rate optimizes mitochondrial density and metabolic substrate oxidation while keeping systemic lactate accumulation below clinical 2.0 mmol/L thresholds. Cognitive ageing is a significant concern in India, where the population of older adults is projected to exceed 230 million within the next decade. With increasing life expectancy, maintaining cognitive and neurobiological health becomes essential for ensuring a good quality of life. This study aims to investigate cognitive ageing in a healthy Indian cohort, providing valuable insights into the trajectories and associations of cognitive, neurobiological, physiological, psychological, and molecular changes across the adult lifespan in India. ## Methods and Analysis This cross-sectional study will collect data from healthy young (19 to 35 years, n = 100), middle-aged (40 to 55 years, n = 100), and older adults (60 to 75 years, n = 100) with regional nativity and residence in Telangana or Andhra Pradesh. Individuals with cognitive impairment or any chronic health condition will be excluded. Assessments will include: ### 1. Standardized Cognitive Tests These tests will evaluate various aspects of cognitive function, such as memory, attention, executive functions, and processing speed. ### 2. Mental Health Questionnaires These questionnaires will assess mental health and well-being, including symptoms of anxiety and depression. ### 3. Brain Imaging Brain imaging techniques, such as functional magnetic resonance imaging (fMRI), will be used to study brain structure and function. ### 4. Heart Rate Variability This measure will assess the autonomic nervous system's response to stress. ### 5. Serum Samples Serum samples will be collected for standard blood tests, including lipid profiles and glucose levels, as well as epigenetic analyses. ### 6. Hair Cortisol Hair cortisol levels will be measured to profile stress. ### 7. Serum Inflammatory Markers These markers will be used to profile systemic inflammation. ## Data Analysis Data from this study will be analyzed using various statistical methods, including regression analysis and machine learning algorithms, to identify associations between cognitive, neurobiological, physiological, psychological, and molecular changes across the adult lifespan. ## Results and Implications The results of this study will provide valuable insights into the trajectories and associations of cognitive ageing in a healthy Indian cohort. These findings will have significant implications for clinical research and health policy, helping to address critical gaps in our understanding of healthy ageing in India. ## Ethics and Dissemination This study was approved by the Institutional Review Board at IIIT Hyderabad (IIITH-IRB-PRO-2023-11). All participants will give written consent. Results from this study will be presented at conferences and published in peer-reviewed journals. Data will also be curated for an interactive web-portal to enable researchers, clinicians, and policy makers to understand the data in meaningful ways. ## Takeaway Understanding cognitive ageing in healthy adults in India is essential for ensuring a good quality of life. This study aims to investigate cognitive ageing in a healthy Indian cohort, providing valuable insights into the trajectories and associations of cognitive, neurobiological, physiological, psychological, and molecular changes across the adult lifespan in India. This study is a significant step towards understanding cognitive ageing in India. The findings of this study will have significant implications for clinical research and health policy, helping to address critical gaps in our understanding of healthy ageing in India. However, it is essential to note that this study has a small sample size and is cross-sectional, which may limit the generalizability of the findings.
| Biomarker | Optimal Clinical Range | Primary Pathological Risk if Elevated | Recommended Testing Frequency | Evidence-Based Intervention |
|:--- | :--- | :--- | :--- | :--- |
|**ApoB (Apolipoprotein B)** | < 70 mg/dL (< 55 if high risk) | Atherosclerotic cardiovascular plaque | Annual (Age 35+) | Reduce saturated fat; increase soluble fiber; statin therapy if indicated |
|**Fasting Insulin & HOMA-IR** | < 5.0 uIU/mL / HOMA-IR < 1.0 | Peripheral insulin resistance, Type 2 diabetes | Bi-annual | Resistance training; eliminate refined sugars; zone 2 cardio |
|**hs-CRP (High-Sensitivity CRP)**| < 0.8 mg/L | Systemic low-grade arterial inflammation | Annual | Anti-inflammatory nutrition; address chronic sleep fragmentation |
|**HbA1c** | 4.8% – 5.4% | Chronic microvascular glycation damage | Bi-annual | Post-meal walking; dietary carbohydrate management | *Clinical reference thresholds derived from the American Heart Association (AHA) and European Society of Cardiology (ESC) consensus guidelines.* ### Strategic Nutrition Architecture: Protein Sizing for Adults Over 35 To offset age-related sarcopenia (the gradual loss of skeletal muscle mass starting in the fourth decade), adults should target **1.6 to 2.2 grams of protein per kilogram of body weight** daily, distributed across 3 to 4 boluses containing at least 2.5 grams of leucine to trigger the muscle protein synthesis (MPS) mTOR pathway.
---
### Long-Term Physiological Risk and Recovery Architecture 1. **Allostatic Load & Cortisol Management:** Chronic psychological and physical stress elevates resting cortisol levels, blunting growth hormone release and impairing muscle protein synthesis. Integrate daily parasympathetic resets (box breathing, sauna, or active walking).
2. **Sleep Architecture Preservation:** Deep slow-wave sleep is the primary physiological window for cellular repair and glymphatic brain detoxification. Avoid late-night blue-light exposure, alcohol, and heavy meals within 3 hours of sleep onset.
